Registration & Consent Form

Completing this form should take around 5 minutes. Please ensure you provide us with this important info used to best care for your child(ren) while in our care. For the purpose of this form all children and youth between the age of 0-18 will be referred to as child(ren). 

Forms will need to be submitted when the child first enters the program and again when entering grade 7. If any changes are made at any point please let us know.
